Working closely with each patient, we formulate a comprehensive care plan that is easy to read and follow for your patients and their relatives, incorporating the relevant care providers, medical and allied health practitioners.

The Over 75 Health Assessment (Item 707)

A comprehensive health check that takes an hour and is performed in the comfort of your patient’s  home or in the practice using our extensive proforma, which has been reviewed and highly praised by the Divisions of General Practice.

Each assessment includes:

  • An extensive review of your patient’s medical and social history;

  • A full health examination and discussion around mental health status, mood and nutrition;

  • A home safety assessment and medication review.

The service requirements of your patient are identified, with a view to ensuring they can live independently in their homes for as long as desired.

The Aged Care Facility can also utilise the Comprehensive Medical Assessment if your patient goes into hospital.

Each assessment consists of an extensive medical and social history, full health examination and discussion, mental status, mood and nutrition assessment as well as a home safety assessment, medication review and service requirements addressed, so patients can live independently in their homes.

Following the completion of the Health Assessment, the average time required by a GP to go through and discuss findings with the patient is approximately 10 -15 minutes.
